Life Transitions

Changing schools, moving, parents separating, leaving home, figuring out what's next. Big changes are destabilising even when they're positive. Support through them helps.

Big changes need real support

Support for the changes that come with growing up.

Young people go through more major life changes than most people realise, and they're expected to handle them without complaint, because "that's just growing up". But transitions are hard, especially when they're unexpected or involve loss.

For neurodivergent young people, transitions hit differently. Changes in routine, uncertainty about the future, and losing familiar structures can trigger significant anxiety or shutdown. We work with that directly.

What we work on together

What we work on in life transitions

Changing schools or moving

Losing your social network and familiar environment overnight is hard. We help you work through the grief and the rebuild.

In practice: Creating a "what I'm leaving behind" and "what I'm moving toward" list, then working through the feelings that come with each side.

Family changes

Parental separation, blended families, a new sibling or a parent's illness can shift everything you're used to. We work through what that means.

In practice: Designing a weekly structure for the new situation that includes familiar anchors from the old one, so not everything changes at once.

Leaving school

The end of school is often more destabilising than people expect. Loss of structure, uncertainty and pressure all land at once, and we work on how to handle them.

In practice: Identifying what you can control and what you can't in the transition, then putting energy into the first group and building tolerance for the second.

Transitions for neurodivergent young people

Routine changes, new environments, and uncertainty hit neurodivergent brains harder. We work on preparing for transitions and managing the sensory and cognitive load they bring.

In practice: Building a visual schedule for the upcoming change, previewing the new place together, and listing the sensory supports to bring along.

Figuring out what's next

Not knowing what you want to do with your life, and feeling like everyone else does, is its own kind of stress. We help you think it through.

In practice: Mapping interests, strengths and values onto a few possible directions, then choosing one small thing to look into or try this month.

Grief in transition

Every major transition involves some loss, of what was familiar, of who you were, of plans that changed. That grief gets room in sessions.

In practice: Writing a goodbye letter to something that has changed, like a friend group, a routine or a plan, then talking through what comes up.

Cost, eligibility & getting started

Costs, eligibility and where we work

What it costs

Pricing for life transitions counselling follows the NDIS Pricing Arrangements and Price Limits. What you pay depends on the session type and whether it falls on a weekday, evening, or weekend. Sessions are also available privately (self-funded) or through other schemes if NDIS doesn't apply to you. Cost is always confirmed in writing before anything starts.

Am I eligible?

Support through a major life transition usually sits under NDIS Capacity Building, most often Improved Daily Living, with Improved Relationships also relevant where family is part of the picture. If a young person's plan includes that funding, it can usually be used here. A support coordinator or plan manager can help check. Private, self-funded access is also available without any NDIS involvement.

Where we work

We're based in Thomastown and work across Greater Melbourne, including the northern, western, and eastern suburbs. Sessions are also available online, which can help when a move or change makes getting to appointments harder. Some support extends into regional Victoria too.

How to start

Reach out by phone, our contact form, or email. The first conversation is about what you're going through, and you share as much as you want to. From there we match you with a practitioner who suits you, and a first session is usually booked within a few business days.

Changes are easier to handle with support behind you.
